Admission

Assisted living facilities are for people needing assistance with Activities of Daily Retirement HomeLiving (ADLs) but wishing to live as independently as possible for as long as possible.

Our Admissions Team is available to answer all of your questions from 9:00 a.m. to 5:00 p.m. Monday through Friday, or by appointment. In order for us to anticipate your needs before your admission, we highly recommend your loved ones tour our facility. We feel confident that our facility is presentable and staff is available to assist you with your questions and concerns at any time. Tours also can be arranged after normal business hours and on weekends.

 We’ll create a service plan for each individual resident upon admission.  The service plan details the personalized services required by the resident and guaranteed by the facility.  The plan is updated regularly to assure that the resident receives the appropriate care as his or her condition changes.


Here is guide to the steps you need to take to place your loved one.

  • Fill out some papers: Emergency form, release of medical information, etc please do not hesitate to ask for our assistance completing these forms. As a licensed care community we are required to have them.
  • Patient's medical history and physical from the hospital or doctor
  • All insurance and Medicare information
  • List of current medications from the hospital or doctor
  • Assisted Living FacilityA photo of the new resident and copies of their social security card, photo I.D insurance cards, Durable Power of Health, Durable Power of Attorney, DNR or any other legal documents regarding conservatorship or financial responsibility will be needed.
  • Arrange for an assessment of your loved one by one of our authorized staff.
  • Less is better! Many families and friends experience mixed emotions when moving a loved one in a Residential Community. Emotions may range from feelings of relief and security to guilt and fear. Some try to compensate by bringing many things into the resident’s new room. This is not always a good idea.
  • Arrange the move-in day.

Our Personalized Assisted Living approach requires that we:
